Please use this form when requesting anyone on the RHS staff to write a letter of recommendation for you. Fill out all questions. Request, in person, the letter of recommendation and state the reason you have chosen this person to recommend you.

If letter is to be mailed, include completely addressed, stamped envelope. The District will not mail letters of recommendation without postage. You should plan to pick up the letter and take it to the post office yourself. This way you can confirm the date the letter was mailed.
